Morbid obesity is associated with OSA, and the concern would be development of respiratory acidosis from hypoventilation. However,vomiting causes a metabolic alkalosis, so the result is a mixed respiratory acidosis with superimposed metabolic alkalosis. The danger is that the alkalemia will reduce the acidemic stimulus to breathe, worsening the pulmonary insufficiency.
